Do you take insurance?

I am considered an out-of-network provider, I have found this enables me to complete all treatments needed for patients without a restriction from insurance. I understand that most patients do rely on receiving insurance reimbursement. I can provide a receipt for you along with a list of the treatments performed and the payment amount that was given so you can seek possible reimbursement from your insurance company if you wish to do this. 

What forms of payment do you accept?

I accept cash, checks, credit cards, debit cards, HSA (health savings accounts), and FSA (flex spending account)

Do I need a doctor's script/referral to be seen?

You can be seen for a evaluation and start treatments without a script/referral. However, Indiana law requires a script within 42 days of your evaluation.  Providers I am allowed to take referrals from include a physician, podiatrist, psychologist, chiropractor, dentist, nurse practitioner, or physician assistant.

What should I expect for my first visit?

A full medical history will be taken at the first visit along with a review of current symptoms/problems. After taking the history, I will then perform a thorough musculoskeletal examination to find restrictions and limitations. If you are being seen for a pelvic floor related diagnosis I will also perform an internal assessment with consent and patient comfort level. You are welcome to bring a second person with you if you are more comfortable that way. We will discuss the physical findings and together set up a plan of care with your personal goals in mind.
